Nurse Family Partnership
Become a stronger parent and make your home a safe place for your baby to live and play.
The Nurse-Family Partnership is an evidence-based community health program that helps transform the lives of mothers pregnant with their 1st child. Each mother served by the program is partnered with a registered nurse early in her pregnancy and receives ongoing nurse home visits that continue through her child’s 2nd birthday.
Your nurse home visitor offers assistance that helps you before and after your baby is born. Each visit is designed around three goals: a healthy pregnancy and delivery, your baby’s good health and development, and your vision and goals for your future.
After you deliver your baby, the nurse who helped you prepare for all the stages of your pregnancy will still be there for you – through breastfeeding, bathing your baby, and changing diapers. She’ll answer all your questions about pregnancy, and after your baby is born will talk with you about talking to your little one – and about planning goals so that your family has a good future.
Through the Nurse-Family Partnership, you will be connected with a nurse who will visit every week or two during your pregnancy and up until your baby is two years. Your nurse will help you:
- Have a healthy pregnancy and a healthy baby
- Become a stronger parent
- Build a strong network of support for you and your baby
- Make your home a safe place for your baby to live and play
- Get referrals for health care, child care, job training, and other support services available in your community
- Find ways to continue your education and develop job skills
- Set goals for your family’s future and find ways to help you reach them
Participation in the program is entirely voluntary, as with all other Family Visitor Program services. There is no charge for participating in the Nurse-Family Partnership.
Bright by Three
If you have a child, birth to three years old, you can sign up, for a one time visit, to receive an age appropriate kit. Call us to find out more about Bright by Text for four and five year olds.
Partners for a
Do you have questions about labor and delivery, breastfeeding, how to introduce a newborn to your other children, basic child care, child development and community services, but are too busy to enroll in other agency programs? You can receive up to five additional visits after your initial Bright by Three A visit. You choose the topics that you want to learn about, and how you want to space your visits. Services can begin during pregnancy or up to six months after your baby’s birth.
Nurse Family Partnership
If you are pregnant, having your first baby, you may choose to enroll this program provided by registered nurses. They cover all aspects of labor, delivery, breastfeeding, basic child care, and child growth and development. Visits begin in pregnancy and continue until your baby is two years old.
Aspen to Parachute
If you are pregnant or are parenting an infant, regardless how many children you have raised this program may be for you. You can enroll during your pregnancy or up to three months after the birth of your child, and receive services until they are three years old. Visits cover all aspects or parenting, child growth and development.
